About the Role:

As the CPCT Regional Implementation Lead, you will spearhead the regional oversight and change management required to successfully implement CPCTs, collaborating with local teams to revolutionize extended primary and community care.

Regular travel across the district will be required and some regional travel may be required.

Key Responsibilities:

  • Develop and implement a comprehensive change management program with local teams.
  • Foster strong relationships and networks within the primary and community care sector.
  • Drive interdisciplinary teamwork and innovative care processes.
  • Manage budgets effectively to achieve CPCT outcomes.
  • Ensure robust clinical governance and facilitate data sharing.
  • Champion the implementation of CPCT roles and monitor progress.

Experience and Knowledge Required:

  • Deep understanding of the New Zealand healthcare landscape and primary/community care organizations.
  • Proven skills in project management and change leadership.
  • Excellent interpersonal and communication abilities.
  • Relevant tertiary education.
  • Valid driver's license required.
