Job description

Introduction: This role is based in the Community working closely with Cancer Patients, you would be delivering exceptional care and support to communities across Whangarei. Position Overview: They are on the hunt for a dedicated and compassionate Community Nurse to join their dynamic team. As a Community Nurse, you will play a pivotal role in delivering high-quality healthcare services to patients within their homes and community settings. You will have the opportunity to make a meaningful difference in the lives of individuals and families by providing comprehensive nursing care, education, and support.

Responsibilities:

  • Conduct comprehensive assessments of patients' health needs and develop individualized care plans.
  • Deliver skilled nursing care, including wound care, medication administration, and disease management, in accordance with established protocols and standards of practice.
  • Provide patient education on health promotion, disease prevention, and self-management techniques.
  • Collaborate with interdisciplinary healthcare teams, including physicians, therapists, and social workers, to coordinate and optimize patient care.
  • Monitor patients' progress, assess outcomes, and modify care plans as needed to ensure optimal health and well-being.
  • Advocate for patients' rights and facilitate access to healthcare resources and support services.
  • Maintain accurate and up-to-date patient records and documentation in compliance with regulatory requirements and organizational policies.
  • Participate in ongoing professional development activities to enhance clinical skills and knowledge.

Qualifications:

  • Registered Nurse (RN) license in New Zealand.
  • Bachelor of Science in Nursing (BSN) preferred.
  • Minimum of 2-3 years of clinical nursing experience, preferably in community or home health settings.
  • Strong clinical assessment and critical thinking skills.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a collaborative team.
  • Compassionate and empathetic approach to patient care.
  • Valid driver's license in New Zealand.
