About the Role:
We are seeking a dedicated Health Improvement Practitioner to join our clinical team. This role involves working within general practices to provide brief, consultation-based services aimed at improving the mental and physical health of individuals, groups, and families across all ages. You will deliver evidence-based behavioural interventions while actively engaging in a collaborative, integrated model of care.
Key Responsibilities:
- Collaborate as a team member within general practices, offering support and interventions for mental and physical health conditions.
- Implement the 'Stepped Model of Care' to facilitate seamless transitions between services.
- Work with other Health Improvement Practitioners and trainers to refine services tailored to the New Zealand context.
Requirements:
- Registered health professional (e.g., psychologist, nurse, occupational therapist, social worker) with a current practicing certificate.
- Minimum of two years' clinical experience in a mental health setting, providing evidence-based psychological interventions.
- Strong communication skills, a flexible and collaborative approach, and a commitment to cultural sensitivity.
Desired Skills:
- Experience with behavioural medicine and an understanding of psychotropic medications.
- Familiarity with working with diverse populations, including Māori and Pasifika communities.
